Reason for referral, including indicated tooth numbers
Referring doctor's name and referring practice name
Any additional notes or clinical details that may help us optimize patient care (for example medical considerations, radiographs sent, urgency, or treatment preferences)
Providing complete information helps us ensure a seamless experience for both your team and your patient.
